Year after year, the nursing homes scored poorly on federal and state assessments. Some have beenfined tens of thousands of dollars for violations. Yet none have faced the kind of consequences that could force them to make changes, complained the Office of the State Comptroller in a report last week. Acting State Comptroller Kevin D.
